Punjab Medical College: Admission to Diploma in Ophthalmic Medicine

Punjab Medical College, Faisalabad has invited applications from candidates for admission in the following postgraduate medical course for the year 2010-2011.

— D.O.M.S (Diploma in Ophthalmic Medicine) (duration: 2 years)

The following are the guidelines for applicants to DO.MS course.

1. The candidate should apply to the Principal, Punjab Medical College, Faisalabad on the printed forms on the payment of Rs.200 available at the Account Section of Punjab Medical College, Faisalabad.

2. Forms can also be downloaded from the website of PMC, Faisalabad. These applicants have to deposit pay order/demand draft of Rs.200 in the favor of Principal, PMC, Faisalabad along with application form. Applications can be sent to the Admission Cell through courier services.

3. Government employees should submit their applications through proper channel on prescribed forms and submit the advance copy of admission form with the official approval of Principal / Chief Executive/Head of Institution directly to the Principal, Punjab Medical College, Faisalabad to be submitted in the Admission Cell of Punjab Medical College, Faisalabad.

4. All rules and regulation will apply as per Punjab Government deputation policy for Punjab Government employees.

5. Others who apply will be considered as private applicants. Private candidates should send/submit the application forms directly to the Admission Cell PMC, Faisalabad.

6. One admission form will be considered only for one course.

7. All the applications should be accompanied with attested copies of the following.

i. Matriculation and F.Sc / B.Sc. / M.Sc. certificate.

ii. M.B.B.S.M.D (Equivalence Certificate of PMDC should be attached).

iii. Certificate from the Principal of the Medical College stating the number of attempts and marks obtained in each professional examination of M.B.B.S/M.D (Attempts Certificate).

iv. House Job certificate with exact date from the concerned Medical Superintendent as well as Professor / Head of the Department of concerned Hospital (Both are necessary).

v. Service Certificate (relevant experience as M.O./Registrar)from the concerned Medical Superintendent as well as Principal/Professor/Head of the Department of concerned Hospital Medical College.

vi. Rural area service certificate. Army service certificate with exact dates and places where served, from the concerned Director Health Services/Competent Authority respectively.

vii. Valid registration certificate of PMDC.

viii. Domicile Certificate.

ix. Four Passport size photographs duly attested (one front and three back).

x. First appointment letter from the Government of the Punjab, Health Department, Lahore as Medical Officer on regular basis.

xi All the documents/certificates should have official stamp dispatch No. and date of tie concerned office / College / University / Hospital,

xii. Certificate from the Controlling Officer that the candidate is Punjab Public Service Commission selectee and no disciplinary action is pending against the candidate.

8. Applications with incomplete documents will not be considered for admission.

9. Following is the criterion for the admission in D.O.M.S. course for the session 2010-11

Preference No 1

a. 6 Months Medicine and Allied as House Officer / Medical Officer from recognized institution

b. 6 Months Surgery and Allied as House Officer / Medical Officer from recognized institution

c. 6 Months Ophthalmology as House Officer / Medical Officer from recognized institution.

Preference No 2

a. One year experience in Ophthalmology as House Officer / Medical Officer from a recognized institution.

or

b. Six months experience in Ophthalmology and Six months in General Medicine or surgery as House Officer / Medical Officer.

10. Eligible candidates will be called for interview along with original documents. No TA/DA will be given.

11. Selected candidates will deposit course fee of Rs.10,000 per annum and University Registration Fee (Rs.1,500) along with the call letter and the affidavit.

12. Admission is liable to be’ cancelled during the course if any discrepancy be found in submitted documents.

The information regarding conditions of admission in above mentioned course will be supplied to the candidate along with application form.

Last date for the receipt of the applications is Tuesday, August 10, 2010. No application will be entertained after due date.
